Today it's possible to use Octopus to promote releases between completely isolated Octopus servers - you can see some approaches in our documentation on working with isolated Octopus servers. However, to make it work requires using our REST API and isn't as smooth as it could be.
In 2018 we plan to make promoting releases between isolated Octopus servers a first-class feature. For an overview of our plans, check out our public RFC on plans for Remote Release Promotions in Octopus.
